Live-In Exemptions Update
HFS has determined that live-in exemptions will require the Department of Human Services – Division of Developmental Disabilities ELECTRONIC VISIT VERIFICATION – (EVV) LIVE-IN CAREGIVER ATTESTATION.

SUBMIT EVV LIVE-IN CAREGIVER ATTESTATION
1. Complete form signed by Employer and PSW (ACES$ will complete the “Provider/Agency” section)
2. Proof of residency documentation for the Consumer
3. Proof of residency documentation for the PSW
Completed forms with documentation for both the PSW and Consumer should be submitted by email, fax or mail.
