Proudly Providing Service Coordination to Consumers on the Aging Waiver
Helping Older People Achieve Independent Living Since 1988
Since 1988, MyCIL (Northeast PA Center for Independent Living) has been helping older people live their best, independent lives. MyCIL is approved by the Office of Long-Term Living in the State of Pennsylvania to provide Aging Waiver Service Coordination.
As a Service Coordination provider, MyCIL serves Consumers on the Aging Waiver in the following PA counties:
- Lackawanna
- Luzerne/Wyoming
- Bradford/Sullivan/Susquehanna
- Wayne
- Pike
- Monroe
All the Aging Waiver services that MyCIL (Northeast PA Center for Independent Living) Service Coordination manages on your behalf are paid through the Medicaid Home and Community Based Waiver Program from the PA Department of Human Services.
How to Choose MyCIL (Northeast PA Center for Independent Living) Service Coordination as Your Service Coordination Entity
The Referral Process
To get the community-based services in your area that will ensure you can live independently — in your own home — you’ll need to complete the referral process:
1. Call MAXIMUS, the identified PA Independent Enrollment Broker, at 877-550-4227.
2. MAXIMUS will provide you with a PA 600 to complete. This form determines your financial eligibility for the Aging Waiver program. You must complete this form and return it to MAXIMUS. MAXIMUS will forward it to your local Department of Human Services/County Assistance Office along with the mandatory verification.
3. MAXIMUS will provide your Primary Care Physician with a medical form to complete. On this form, your physician will recommend your Level of Care.
4. After MAXIMUS receives the completed medical form, they will contact your local Area Agency on Aging (AAA) to request a Level of Care determination.
5. An assessor from your local AAA will contact you to schedule a home visit for your Level of Care assessment.
6. Once the assessor completes the Level of Care assessment, your AAA will forward a Level of Care determination to MAXIMUS.
7. If the Level of Care determination indicates that you have been assessed as Nursing Facility Clinically Eligible AND you are financially eligible, MAXIMUS will contact you to arrange a home visit. During this home visit, you’ll choose a Service Coordination Entity.
Your Initial Home Visit With MyCIL (Northeast PA Center for Independent Living) Service Coordination
Getting Started
When you choose MyCIL (Northeast PA Center for Independent Living) Service Coordination as your service coordination provider, MAXIMUS will send us your referral. Within five days of receiving this referral, we’ll call you to schedule a home visit.
During your initial home visit, your service coordinator will help you:
- Define your needs, challenges and independent living goals
- Review your eligibility for additional Medical Assistance benefits and guide you through the application process for those you qualify for
- Develop an Individual Service Plan (ISP) to meet these challenges and goals
Once the Office of Long Term Living approves your ISP, your MyCIL service coordinator will connect you with the right community-based services in your area. From home modifications and durable medical goods to in-home assistance, you’ll receive the supports you need to reach your independent living goals.

Advocating for You
Connecting you with the right community-based services in your area is just one aspect of what we do for you. We also continually review the services you receive, advocating on your behalf to ensure you’re receiving the level of support you deserve.
Your dedicated service coordinator will:
- Meet with you in-person periodically to review your strengths, needs, preferences, supports and goals
- Update your ISP as needed to continue reaching your independent living goals
- Track the services you’re receiving to ensure they meet your identified needs
- Advocate for you to ensure your health and welfare
